Learning from Experience: Interactive Cases in Patient Safety

Lisen Schortz, MSc, Patient Safety Manager at AniCura and Dr. Kathrine Blackie, Quality Improvement Manager at Linnaeus will outline how to learn from patient safety events. Dr. Blackie and Samantha Thompson, RVN GradDipVN CertVN ECC DET TAQA, Medical Quality Manager at Linnaeus, will work through interactive real case examples, so delegates can practice what they have learned and come away with strategies that can be used in practice, starting with the following basic principles:
- Avoid blame. If the goal is learning and improving, pointing out who was at fault won’t fix anything, and it may create a culture of concealment and fear.
- Adopt a systems approach. Think about what was to blame rather than who was to blame, and how controls contributed to the outcome.
- Identify contributory factors. External stressors and how people behave are situational and often complicated.
- Be aware of your cognitive biases. Hindsight bias mean you know the outcome and can often more easily identify contributors. Outcome bias means we tend to view an action in a more negative light if things didn’t go as planned.
- Accept that learning means changing. Understanding the event, knowing where things went wrong, and recommending action is fine, but it is not meaningful if change is not implemented.
